21.婦人科・産科領域への配慮

先天性角化不全症およびテロメア障害（DC/TBDs）の女性は、婦人科的および生殖器系の健康に配慮が必要な場合がある。この章では、日常的な婦人科領域の検診、HCT（骨髄移植）に関する特別な懸念、婦人科系の状態、妊孕性、出生前診断、妊娠および産科合併症に関して検討する。DC/TBDの女性は、婦人科医を臨床ケアチームに加えることで、婦人科的および生殖器的健康状態を評価することができ、有益である。

概要

　最近発表されたGiriらによる、39名のDC/TBDs患者の初経後の婦人科的問題、妊孕性、妊娠に関する報告によると、彼女たちは一般集団と比較して通常の年齢で初経と閉経を迎えるにもかかわらず、いくつかの重要な婦人科的問題に直面しています [1]．
　まず、女性の3分の2が血液学的な症状を呈し、そのうちのかなりの割合が、ホルモンによる避妊をしているにもかかわらず月経が重いままであることが特徴です[1]。
　第二に、これらの女性の一部は、後に骨髄不全または悪性腫瘍の治療のために造血細胞移植（HCT）を受ける可能性があります。HCTは、婦人科と生殖器の健康について独特の懸念があり、多くの場合、早期の閉経または不妊につながります [2] 。さらに、患者がHCTを受けた場合、治療による二次がんのリスクと子宮頸がんのリスクが増加しますが、子宮頸がんのリスクは、定期健診と移植後のHPVワクチン接種により増加しないようです［3, 4, 5］。
　第三に、DC/TBDsの女性が妊娠するにつれて、その妊孕性と妊娠結果に関する情報が明らかになってきています。他の遺伝性骨髄不全症候群の女性と同様に、妊孕性に問題があり、低血球数および血小板数に関連する妊娠合併症の発生率が高い可能性があります [1、6-8]。
出産を希望する女性のほぼ全員が妊娠し出産したが、妊娠するために不妊治療補助を利用しなければならないこともあり、受胎能力低下の兆候である習慣性流産の確率も高かった[1]。最近の報告では、反復流産のリスク増加、妊娠中毒症、早産や初回帝王切開率の上昇につながる血液学的合併症の懸念から、DC/TBDsの女性は血液学、婦人科学、母体胎児医学の専門医による協調的なケアが有益である[1]。
　第四に、テロメア短縮を有する一般女性は、40歳未満の閉経や早発卵巣機能不全、不妊のリスクが高いことが知られていますが、DC/TBDsを有する女性の最近の研究では、中央値49歳での自然閉経が明らかになっています [1, 9]。女性は一般的に50代まで生存しており、生殖年齢を通じて必要なスクリーニング検査を受けることの重要性が強調されています [10]。DC/TBDsの女性は、あらゆる種類の悪性腫瘍のリスクが高いが、婦人科がんの全体的なリスクは高くないようである [3] 。
